Welcome to EmBody Health! I'm Jen Graff, AGPCNP-BC (Adult Gerontology Primary Care Nurse Practitioner - Board Certified - whew, what a mouthful!) and I look forward to getting to know you and to help you reach your health and life goals together. For your convenience, our appointments are offered by telehealth the majority of the time with a limited amount of in-person visits per year, including your annual wellness exam. These will be done in the comfort of your own home, where you are relaxed and away from a crowded waiting room full of sick patients. My visits are scheduled for 30 minutes to one hour long (depending on need) so that you get the time and attention you deserve.
EmBody Health is a boutique practice. We do not take insurance because I firmly believe that administrators at an insurance company do not have your best health goals at heart and have no business dictating your healthcare.
