Our history
EmBody Health, LLC is owned and operated by Jennifer Graff, AGPCNP-BC, who has been a trusted provider of high-quality healthcare since 2016. I started this practice after having several of my former primary care patients complain that they don't feel heard or understood by many of their healthcare providers, and that there had to be "a better way" to receive the care they desire. EmBody Health is my response to this need and my passion to give the healthcare that we all deserve.
Our mission
My passion has always been caring for my patients like members of my own family. I'm here to offer a new experience in healthcare - adding world class individualized medical care and attention to your goals so that you succeed in all aspects of your life, not just with lab values and tests. My training and subsequent practice as a nurse practitioner has allowed me to EmBody the mindset that you are more than a sum of body parts, and you are definitely more than a quick 15 minute office visit.
